SITUATIONS THAT REQUIRE A GAME REPORT

Situation: Game Misconducts (Including Instigator and Aggressor penalties)
Action Required:  

  • Fill in the back of the original game sheet. Record the time, period, player’s name, number and team, penalty and rule reference. Sign the game sheet. There are no details required.

Situation: Trash Talking Rule 9.2F
Action Required:  

  • Fill in the back of the game sheet
  • Keep the original game sheet (the white copy)
  • Complete a Game Report
  • Email, phone or fax the report to the appropriate person by noon the next day
  • Mail the game sheet and game report within 24 hours of game completion

Situation: Game Misconducts Rule 9.2A, 9.2B(2)(3)
Action Required:  

  • Fill in the back of the game sheet
  • Keep the original game sheet (the white copy)
  • Complete a Game Report
  • Email, phone or fax the report to the appropriate person by noon the next day
  • Mail the game sheet and game report within 24 hours of game completion

Situation: Major Penalty for Checking from Behind or Head Contact
Action Required:  

  • Fill in the back of the game sheet
  • Keep the original game sheet (the white copy)
  • Complete a Game Report
  • Email, phone or fax the report to the appropriate person by noon the next day
  • Mail the game sheet and game report within 24 hours of game completion

Situation: Gross Misconducts
Action Required:  

  • Fill in the back of the game sheet
  • Keep the original game sheet (the white copy)
  • Complete a Game Report
  • Email, phone or fax the report to the appropriate person by noon the next day
  • Mail the game sheet and game report within 24 hours of game completion

Situation: Match Penalties
Action Required:

  • Fill in the back of the game sheet
  • Keep the original game sheet (the white copy)
  • Complete a Game Report
  • Email, phone or fax the report to the appropriate person by noon the next day
  • Mail the game sheet and game report within 24 hours of game completion

Situation: Unusual Situations – line brawls, player(s) leaving bench, refusing to start play, serious injury, etc.
Action Required:  

  • Fill in the back of the game sheet
  • Keep the original game sheet (the white copy)
  • Complete a Game Report
  • Email, phone or fax the report to the appropriate person by noon the next day
  • Mail the game sheet and game report within 24 hours of game completion

SPECIAL NOTE:
For Rules 9.6 (Physical Harassment of Officials) and 9.7 (Spitting involving an official), copies of the Game Sheet & Game Report are to be emailed or faxed AND mailed to the SHA Office (Attn: Manager, Officiating) regardless of the type of game (Exhibition, Tournament, League, Provincial).

Reports are to be submitted in the following manner:

LEAGUE GAMES:

  • Email, fax, or phone the League President by noon the following day
  • Mail the game sheet and game report to the League President within 24 hours

SHA PLAYOFF (PROVINCIAL) GAMES, TOURNAMENTS, and EXHIBITION

  • Go to SHA website and file online or fax Game Report & Game Sheet to 306-789-    6112 (Attn: Manager, Officiating) by noon the following day

Rule 9.6 Physical Harassment of Officials & Rule 9.7 Spitting (involving an official)

  • These apply to League, Exhibition, Tournament, and SHA Playoff (Provincial) games
  • Email or fax the game sheet and game report to the SHA Manager, Officiating (TrentC@sha.sk.ca or 306-789-6112)
  • Mail game sheet and game report to the SHA Office (Attn: Manager, Officiating)

SPECIAL NOTE:

  • If you have any trouble filling out a game report or require some assistance please contact the Manager, Officiating @ 306-789-5101 or any SHA Referee’s Division Executive member.
  • Reports are to be neat and have sufficient detail without getting wordy.
  • The Referee(s) is/are responsible for submitting all reports.
